The CASE form system has a dedicated dashboard for each user account that provides access to your sent forms, to access the dashboard for the first time you will need your credentials that were emailed to you when you sent your first form.

If you have sent a CASE form but not received your login details then please email caseforms@activisthub.co.uk so we can look into this for you.

Step 1

Head to https://case.activisthub.co.uk and click Log In.

Step 2

Enter your Email Address (1) and Password (2) then click 'Login', if you have forgotten your password then click 'Reset Password' and follow the steps to create a new one.

If you have entered your details correctly you should be taken to the Dashboard which will look similar to the below image.
